Cheque Request Form* - Need to make a payment from your club account?
Budget Request Form - need a helping hand with a cost? If you don't ask you don't get
Change Of Signatory Form* - if you resign from your post the new commitee need to record their details.

Room Booking Form - need some space? We can book a room for your requirements     
Risk Assesment Form*- any event/activity will need some admin, just to keep you safe.
Accident Form* - Accidents do happen. We just need to record the details.

Minibus                                                               

 To drive a minibus, you must first have passed the minibus assessment run by the Student Union. To take the test you must be a student at Bournemouth University over  the age of 21 with a clean licence who has been driving for 2 years. Please collect the insurance form from the Student Union reception with both parts of your clean driver's licence.

Other essential forms                                       
Table Booking Form - want to advertise yourself? Grab a table in the Atrium for some exposure
Inventory/Equipment Log Form - any new equipment you have needs to be recorded for insurance
Development plan- to be completed by all clubs and societies every year at the end of the year
Safe six guide - to help manage the health and safety of your club/soc
 



*IMPORTANT NOTE: These forms all require a signature(s) in order to process them, please ensure that they are signed by correct memebers before submitting!!!

** IMPORTANT NOTE: All drivers must have completed minibus test and insurance form before this form can be processed.
